6. Are you a church, school, hospital, credit-counseling organization, supporting organization, or other specially listed organization?
7. Do you plan to maintain a donor-advised fund, participate in certain joint ventures, or conduct activities that require specialized IRS review?
8. Has tax-exempt status previously been revoked or are you applying as a successor to a revoked/suspended organization outside the allowed reinstatement paths?
